If you have a complaint about the quality of care or other services you have received, you can file a grievance. Please follow these steps:

  • Complete the Grievance Form below.

To appoint a representative, please download and complete the CMS Appointment of Representative Form (CMS-1696). Once completed, send it to our office at the address provided on the form.
